Operator Authorization
ONLY trained and authorized operators may operate forklifts. LEO requires documented training and evaluation per OSHA 29 CFR 1910.178. Authorization is site and equipment-specific. Refresher training is required every 3 years or after any accident, near-miss, or observed unsafe operation.
Pre-Use Inspection
Complete a documented pre-use inspection every shift: fluid levels, tires, forks (cracks, bends), mast, chains, overhead guard, load backrest, lights, horn, brakes, steering, seatbelt, and data plate. Do not operate a defective forklift — tag it out and notify your supervisor.
Stability Triangle & Loads
The forklift's stability depends on the stability triangle (front axle and rear wheels). Loads must be: centered on forks, carried 6 inches above ground while traveling, tilted back for stability. Never exceed the rated capacity shown on the data plate. Capacity decreases with load height and fork spread.
Pedestrian Safety
Forklifts and pedestrians are the #1 source of PIT-related fatalities. Establish pedestrian lanes. Operators must slow and sound horn at intersections and blind corners. Pedestrians have the right-of-way. Maintain 3-vehicle-length following distance. Never carry passengers. Keep clear of 10-foot swing radius at all times.
